This patient wanted to fill out the volume that she lost with having kids. Because she has sagging redundant tissue in the bottom half of her breasts, we opted to place her silicone implants above the muscle so the implants can drop down and fill out her breasts. There are some occasions that call for placing the implants above the muscle, and this is one particular case. If we had place her implants in the submuscular pockets, we could run into the possibility of having the ‘waterfall’ effect, which is when the breast tissue hangs over the implants held up by the chest muscles.
